Once you download Neo Cosmos from itch.io, you can follow this instruction to install it and play it. Please note that you have to own A Dance of Fire and Ice to play this DLC.

Because of macOS security measures, apps downloaded from websites like itch.io may have limited behavior by the operating system, including loading Neo Cosmos properly. We heavily recommend downloading both ADOFAI and Neo Cosmos from the itch.io official app to make this work! Otherwise you may get unintended errors. Otherwise you may get unintended errors that can lead of ADOFAI not recognizing Neo Cosmos.

It is for Windows but for Mac or linux, it is pretty much the same!
On Windows:
neo-cosmos-win.zip in a folder called Bundles.Bundles folder in the same folder where A Dance of Fire and Ice.exe is located.On Mac:
